Obagi acne treatment and sunscreen? Which one first?

I’ve been using Obagi Clenziderm system and I started using sunscreen. I was wondering if I’m supposed to apply the sunscreen before or after the therapeutic lotion (which is 5% Benzoyl Peroxide.)

What Can I Do About My Girlfriend’s Therapist If She Charges For "No" Treatment?

This is somewhat long, but very serious. Please tell me what you know I should do.

For about six months, my girlfriend’s therapist is charging her just to go in and lay down on the couch, sometimes to say nothing at all!

Unfortunately, some patients with depression, are unable to speak their mind and can by manipulated by people very easily, because they lack assertiveness.

My girlfriend doesn’t make much money, and I think she spends about 70% of her income on "treatment" twice a week, which is 30 miles away, which obviously takes a hit on gas money.

The therapist has only been practicing for a year or two, I think. This therapist charges a high amount. She see’s the therapist twice a week for 50 minutes each.

In therapy, the therapist is critical, judgmental, and get’s mad at her for her behavior and stuff like that. She tells her that she has a "distorted" perspective of reality. Which I don’t think is necessarily true, since a majority of depressed people have a negative explanatory style. She also has an unprofessional vocabulary, she has called me "retarded," because I have PDD-NOS (Pervasive Development Disorder – Not Otherwise Specified) and I have General Anxiety Disorder.

Although my girlfriend feels more comfortable, the therapists tries to behave and speak as if they were good FRIENDS. I am certain this violates the ethics of CAMFT and AAMFT.

There are many times when my girlfriend simply has nothing to talk about, and so the therapist "BELIEVES" in NOT asking questions. What kind of therapist doesn’t ask questions? How did she even reach a diagnoses without asking questions? Treatment is unhelpful and inefficient by NEGLECTING the patient.

So she will sit there, do NOTHING for 50 minutes, sometimes both days of the week, and she will still be charged and not asked if she would like to leave early. I know many therapists who will allow their patient to leave if nothing can be discussed at NO charge.

This is very unprofessional and unethical, in my opinion. The reason it is unethical is because she is taking advantage of my gf’s condition, which is depression. Unfortunately, some patients with depression, are unable to speak their mind and can by manipulated by people very easily, because they lack assertiveness.

I wonder if this therapist is doing this to anyone else. I think it is necessary for the therapist to be reviewed by a license/certificate provider, like CAMFT and AAMFT. Unfortunately, I am a third party and cannot do much except try to convince my gf to stop seeing her. However, my gf is reluctant to do that because she doesn’t want to "explain" everything again to someone else.

Ethical Standards in Question:

1.2 Marriage and family therapists are aware of their influential position with respect to patients, and they avoid exploiting the trust and dependency of such persons. Marriage and family therapists therefore avoid dual relationshipsiv with patients that are reasonably likely to impair professional judgment or lead to exploitation. A dual relationship occurs when a therapist and his/her patient engage in a separate and distinct relationship either simultaneously with the therapeutic relationship, or during a reasonable period of time following the termination of the therapeutic relationship. Not all dual relationships are unethical, and some dual relationships cannot be avoided. When a dual relationship cannot be avoided, therapists take appropriate professional precautions to insure that judgment is not impaired and that no exploitation occurs.

1.3.1 – Marriage and family therapists terminate therapeutic relationships for clinically sound reasons and in an appropriate manner. Reasons for termination may include, but are not limited to, the patient is not benefiting from treatment, continuing treatment is not clinically appropriate, the therapist is unable to provide treatment due to physical or mental illness, or the treatment becomes ethically questionable.

1.3.3 – It is unethical to maintain therapeutic relationships solely for financial reasons.

1.6 – Marriage and family therapists continue therapeutic relationships only so long as it is reasonably clear that patients are benefiting from the relationship.

1.8 – Marriage and family therapists do not abandon or neglect patients in treatment. If a therapist is unable to continue to provide care, the therapist will assist the patient in making reasonable arrangements for continuation of treatment.

1.16 – When a marriage and family therapist is not located in the same geographic area as the patient, he/she shall provide a name of another qualified mental health care professional and/or entity in the patient’s locale for contact in case of emergency.

3.10 – Marriage and family therapists do not assess, test, diagnose, treat, or advise on problems beyond the level of their competence as determined by their education, training and experience. While developing new areas of pract

I would get a hold of the State Medical Board and ask them who the agency board is that does licensing for Marriage and Familly therapists in your State.

Then, I would recommend asking for general information about what the process is for if someone should decide to file a complaint with the licensing board.

treatment for gerontophobia (fear of old people)?

I’m trying to find a behavior therapy that would be best for treating a fear of old people

One treatment option is exposure treatment. this is where the patient is exposed to the phobic stimulus as part of the therapeutic process. (Such as someone having a fear of snakes, and being exposed to many non-poisonous snakes for an extended period of time until the extinction of such phobic response is possible)

Another option is flooding, where the preson is immersed in the fear reflex until the fear itself fades away.(Soe phobic reactions are so strong that flooding must be done through one’s imagining the phobic stiulus, rather than engaging the phobic stimulus itself.

Counter-conditioning- one is trained to substitue a relaxation response for the fear response in the presence of the phobic stimulus. Relaxation is incompatible with feeling fearful or having anxiety, so it’s said that relaxation response counter’s the fear response.

Systematic desensitization involves 3 steps 1) training the patient to physically relax, 2) esxtablishing an anxiety hierarchy of the stimuli involved, and 3) counter-conditioning relaxation as a response to each fear stimulus beginning first with the least anxiety-provoking stiulus and moving then to the next least anxiety-provoking stimulus until all of the items listed in the anxiety hierarchy have been dealt with successfully.

which method do you think would be the best in treating a fear of old people? I’m thinking either counter-conditioning, or systematic desensitization but I’m not sure :(

well..actua;lly we all will be old one day..and this is good..we will be wiser and so God made the world to become grandparents..etc..actually..we must help the old people bring them smth cooked..buy them smth nice..talk to them..make them a good day..and we can also hear usefull things..
we also must honour our parents garndparents etc..is God’s comandement.so we must try to fullfil as we can…God will also give us what we need to do HIs will..and respect the people.

even if sometimes they say smth that disturbs us..we can tell them that things are not just like that..that so and so..that this is the opinion..that there are other views..that GOd says this and this..and actually i don’t let myself influenced neither by younger people nor of my age nor of old if i what they say is not ethic, doesn’t fit the common sens and God’s principles

Medical Treatment of Special Needs children with an EX (physician) denying medical treatment.?

I am divorced 8 years from a Doctor. I have full custody of our 3 special needs kids, but I am supposed to agree on all treatment for medical/educational etc. We have extensive documentation that he has tried to deny medical treatment, well child exams, any therapies, Has not paid for educational, medical or extracurricular activities for three years.Last year my daughter testified from a wheel chair because he filed an injunction to stop her surgery. Yes she got the surgery, and thank GOD is walking 3-4 miles a day. This has been going on for years. Even though he is obligated to pay, I have had to pay for all the services so that my children would have appropriate treatment and care.
Now my autistic son has become violent and he refuses to allow psychiatric treatment. He has spent one night in a psyche hospital, stabbed a kid at school, increased his aggressive behaviors and I want an inpatient evaluation at a residential hospital. II have three psychiatrists recommending inpatient treatment including his pediatrician. I WANT my son to live with me, but he becomes violent and want the meds leveled out. The ex (physician) is now trying to take him away from me. In his care, he lets him watch movies like PSYCHO, The BIRDS and other horror. He acts out these characters and is obsessed with TV. He is 15 years old, 200 pounds and the mentality of a 5 year old. His father lets him watch TV for 8-12 hours a day unsupervised. At my home he watches 1-2 hours a day of approved and limited programs. He has reduced his parenting time significantly. Sees him every 3-6 weeks.
We go to court soon (my request) to mandate the intervention. The judge has admonished him several times but I am very concerned. If the judge awards him custody (he is doing this now because he doesnt want to pay child support or medical bills), it will be to the grave detriment of my son. He buys him everything, and when he comes home my son threatens to go with his dad, call him constantly, and gets violent. Its a 5 year olds tantrum in a 200 pound 5 foot ten body. I get the behavior modification techniques, and hold a masters in special education. You dont see behaviors from my son unless you make him follow his schedule, do his chores, homework, and life skills and social skills activities. (by the way, he loves the feeling of accomplishment when he does complete the tasks-all of which he CAN do.)
He has bled me dry financially, owes me about 30K from bills he refuses to pay, (not including the 20K I have spent on lawyers and does not pay me NEAR the 41% of the 500K he makes a year. Yes I have been trying to take him to court for 3 years to adjust support. My children have had to testify in court on other issues, and I have medical reports that show my sons psychosis, hallucinations, and threats to his life and others.
One friend of mine said the other day, I dont know how you even get out of bed every day. I have been fighting for medical, educational and therapeutic services since my children were born. If he is in control, he said he would pay for everything they needed. The problem, when my son is in his care, he watches TV, and EATS till he throws up (constantly), and according to him, he doesnt NEED all that educational and therapeutic STUFF. With a powerful man in a relatively small community, I am up against a lot. I have prayed, and am on so many prayer chains. My house is soon to foreclose, and at times I feel so overwhelmed. I will never stop fighting for my kids, and I do believe God knows my heart, but I am unsure what to do. I cant switch lawyers (he bought off the first two), as I am out of money for a downpayment. I have never posted on a board on the web, but I have tried everything else, so here goes. Any ideas, suggesstions or insight would be so greatly appreciated. Thank you for your time.

I’m not a lawyer and won’t attempt to guess the outcome of custody hearing but what I will suggest is that it’s time to play dirty.
Have a trusted friend call a newspaper and tell them that this physician is not paying for the upkeep of his children and they are about to become homeless etc. Even if they do not run a story, your ex husband will not want a journalist probing his life.
He is playing mind games (controlling behaviour) with you, maybe time to fight fire with fire.

Would a pet help in the recovery from depression?
