1) Digoxin does not alter cardiac output in normal individuals.

2) It does cause significant increase in contractility.

What are the threats to the Physical Therapy profession?

With an increasing number of demand for therapeutic services with a lack of DPT professionals to provide this service how will these demands be met?

More use of PT assistants?
Physiotherapists?
DCs?
Massage Therapists?

Will the trend go in favor of "cheaper" therapeutic alternatives while the DPT profession decreases in value due to their high costs? How will health care reform affect DPT reimbursement in the future? How will it affect the PTA profession?

On the outside it looks as if the PT profession is very bright. But what if there is a more "Universal Health care" system that arises from this economic crisis. Obama has mentioned health care reform as a primary issue. How do you see the PT profession ten and twenty years from now?

I agree with Mistify. I will add that there is an increased demand for PTs as the population ages and the premature infants that are born earlier and earlier. An increase in PTAs are a good thing. PT should be the primary person in musc-skeletal problems.

I do not see PT going away. I think another big threat is ATC used as PTA. This is not legal but happens. PT are getting more efficient with evidence based medicine. Patients are getting better sooner. We also need to empower our paitents to take ownership on their rehab.

Embryonic stem cell research, or, potentially, in regenerative medicine where it is sometimes referred to as "therapeutic cloning." One vision for successful stem cell therapies is to create custom stem cell lines for patients. Each custom stem cell line would consist of a collection of identical stem cells each carrying the patient’s own DNA, thus reducing or eliminating any problems with rejection when the stem cells were transplanted for treatment. For example, to treat a man with Parkinson’s disease, a cell nucleus from one of his cells would be transplanted by SCNT into an egg cell from an egg donor, creating a unique lineage of stem cells almost identical to the patient’s own cells.

Potentially millions of patients could benefit from stem cell therapy, and each patient would require a large number of donated eggs in order to successfully create a single custom therapeutic stem cell line. Such large numbers of donated eggs would exceed the number of eggs currently left over and available from couples trying to have children through assisted reproductive technology. Therefore, healthy young women would need to be induced to sell eggs to be used in the creation of custom stem cell lines that could then be purchased by the medical industry and sold to patients. It is so far unclear where all these eggs would come from. The sale of human eggs is normally referred to as a "donation," but women who donate their eggs are often paid thousands of dollars.

Embryonic stem cell research also brings up a moral issue and could potentially lead to human cloning.

Therapeutic cloning should definitely move forward. I think you are wrong that healthy young women will be encouraged to sell eggs. Any way – I don’t care if they sell eggs.

Why couldn’t eggs be harvested from removed ovaries? Women have hysterectomies every day. I think you are seeing problems that don’t exist.

Can getting a tattoo be therapeutic for joint problems?

My husband got an armband tattoo on upper arm. He had been having shoulder problems, which could have been arthritis or bursitis at the time. He’d been taking different meds with no improvement of the symptoms…until he got the tattoo. Within days the shoulder got better and hasn’t bothered him since. Coincidence or was there something related to the tattoo… I am thinking about the similarities of acupuncture and bee sting therapy I’ve seen on tv. What do you think?

could have been an inadvertant stimulation of a meridian (used in acupunture treatment), if so it will likely come back so you know that his best method of treatment if that occurs would be…. acupunture or chiropractic

Is there a such thing as a Certified or Licensed "Medical Massage Therapist" and I heard of "Therapeutic MassaGE Therapist". what are the differences. How do i get certified or licensed in either one. (I DONT WANT TO BE A REGULAR OR RELAXATION MASSAGE THERAPIST)

I consider myself a medical massage therapist. It goes beyond just rubbing and squeezing muscles. And it is not all accomplished in the massage room either. A medical massage therapist trains and recognizes his/her self to be integrated into the healthcare system; being a viable part in a person’s management or recovery plan as directed by a medical physician or chiropractor. A physician, other healthcare providers, insurance companies and attorneys need medical massage therapists to understand the record-keeping, billing, privacy issues, ethics and, yes, advanced technique training it takes to be considered an ancillary provider within the western, allopathic, medical system. Massage is therapeutic in many aspects, with many different theories, techniques and approaches. But not all of them can fall within the category of medical massage. It is not just a marketing term either. The term was created to force a delineation between those that want to obtain advanced training and practice to evaluate, treat, report findings and summarize progress on patients, and those that do not want to follow that path. In this separation, it is in hopes that the allopathic system finds more focussed and recognizable definitions for necessity of treatments. It is an infant term that is attempting to advance to another level of acceptance.

To answer your question: a massage therapist is either licensed, certified or registered, depending on the regulation agency in your state or municipality. LMT, RMT, CMT are the most common abbreviations for these credentials. There are certifications in medical massage, but it does not necessarily hold a lot of water right now. It just seems to be another title. The most recognizable certification entity for massage therapists is the National Certification Board for Therapeutic Massage and Bodywork http://www.ncbtmb.org/
You can obtain a certification by taking one of their two certification exams. They are even working on another advanced certification for massage therapists. Even though this is considered a national exam, not all states or regulatory agencies recognize these exams for licensing. If you do not want to be a regular relaxation massage therapist, then worry less about the title, and research more on specific continuing education programs that will gravitate you away from the rest who want to remain relaxation massage therapists. Titles are nice, but most people have no idea what they even mean; especially your potential clientele. Market yourself as something different in the way you work with people, the other healthcare providers you associate with, and the quality of service you provide. Find therapists in your area or even out of your area that you would like to imitate, then befriend them. Experienced therapists often like to share their experiences and how they got where they are. If they don’t, then you probably don’t want to be like them anyway. Look for CE courses from David Kent, Whitney Lowe, Ralph Stephens, Paul St. John, John Barnes, Aaron Mattes and topics on orthopedic massage, sports massage, muscle energy technique, myoskeletal alignment technique, manual therapy techniques, trigger point therapy, assisted stretching, myokinesthetic system, active release technique, etc, etc.

Efficacy and toxicity are two key aspects of drug response of clinical relevance.

Efficacy refers to the maximum beneficial or therapeutic response that a drug can produce, and is a measure of clinical effectiveness. It can be expressed in terms of the percentage of recipients who show a therapeutic response at a given, standard dose.

Toxicity refers to the extent to which a drug induces unwanted or harmful health effects, and may be expressed as the percentage of patients who show adverse side effects at a given dose. The optimal dose range for a drug is that at which efficacy is greatest and toxicity is lowest. In practice achieving an acceptable level of toxicity usually means that efficacy is compromised and in some cases, even the safest drugs can produce severe side-effects. Extreme drug responders within a patient population are of concern. These include non-responders in whom the drug is ineffective, and adverse responders in whom the drug causes major harmful side-effects. If it was possible to identify these individuals in advance, treatments could be improved; the non-responders could receive an increased dose or alternative medication, and the adverse responders could receive either a greatly decreased dose or an alternative form of medication.
